New Patients
When you make an appointment at Seven Circles Natural Medicine you can expect to be greeted when you enter and offered water or hot tea. You will meet with the doctor for an hour-long appointment and receive a comprehensive evaluation and treatment, with acupuncture if you'd like. You may receive nutrition and/or lifestyle counseling and, at the end of your appointment, you may be prescribed something from our natural pharmacy.
To schedule your first appointment, please call our office we will get you set up in our system. If you'd like to use insurance please have your card ready, as we will need some information to verify coverage. Please note: we encourage our patients to be familiar with their own policy as you are ultimately responsible when coverage is denied.
Before your visit with us please print out the Adminstrative Paperwork and Medical Paperwork packets to fill out to the best of your knowledge. Bring your completed paperwork and insurance card (if applicable) with you to the appointment. If you don't have access to a printer please come at least 30 minutes early to your appointment to allow time to complete it. Please also eat and drink water before your appointment and if you plan on receiving acupuncture wear loose fitting clothing that can be pushed above the elbows and knees.
If you are taking any prescription medications or vitamins/supplements please make note of them in the New Patient paperwork. Please bring in all supplements, herbs, and/or vitamins you are taking by prescription from another doctor or self-prescribed.
Cancellation Policy: We require at least 24 hour prior notice for all appointment cancellations. Unless there is a legitimate emergency, cancellations made the day of and no-shows will be charged $50 for the first time and the full cost of each missed visit thereafter. This fee can be waived if the next available appointment is taken.
